It's easy, just put the new model and texture in the right folder and the game will load it (because the game supports this in the first place. Not because it's hacked to use it or anything). But only YOU will see the new skin/model. Unless you send it to your friends and they also add it, they can't see it. No matter what server you play on. If they don't have the content, they can't see it. So everyone would need the hacked/modified client.
